Requirements for CE credit: Payment and Registration Form
Each SELF STUDY BOOK has a correlating downloadable Registration form. Please make sure that the Registration form corresponds with the chosen CE course. Complete Sections A and B on the Registration form. Test answers can be documented on Section C of the Registration form. Once completed, email Registration form along with test answers (Sections A, B, and C) to iaahs.edu@gmail.com. Registration form and transcript will be kept on file as documentation of CE hours.
